Applications For Services

Southlake Utilities, Inc. provides water and wastewater services to residential developments, apartment communities, and businesses.

These applications (with the exception of new businesses) can now be printed ahead of time and brought into our office.

Please select the appropriate application you are seeking from our drop-down menu for addition information on setting up services, connection and deposit fees, and enrolling in the electronic payment option (ACH). Applicant must provide proper identification for legal rights to the property to establish service. Please call our office if you have additional questions.

Application for Apartment Home

Welcome to your new apartment home! Please print the form below.

You will need to bring the following information to complete your application:

  • Your welcome letter from your apartment complex.
  • Valid government-issued identification.
  • Optional: A voided check if you wish to enroll in the electronic payment option.

The following connection charges and deposits will apply:

  • Initial Connection Charge (Water and Wastewater) – $15.00
  • Deposit (Water) – $17.50
  • Deposit (Wastewater) – $17.50
  • Total Connection Charges and Deposits = $50.00

Single Family House Application

Please print the form below and bring in the following information. You may email the form to billing@southlakeutilities.com or bring it with you to our office.

For Rental Properties: You will need the following information to complete your application:
• Your lease.
• Valid government issued identification.
• Optional: A voided check if you wish to enroll in the electronic payment option.

For New Homeowners: You will need the following information to complete your application:
• Closing documentation or HUD statement.
• Valid government issued identification.
• Optional: A voided check if you wish to enroll in the electronic payment option.

The following connection charges and deposits will apply:

  • Initial Connection Charge (Water and Wastewater) – $15.00
  • Deposit (Water) – $35.00
  • Deposit (Wastewater) – $35.00

Total Connection Charges and Deposits = $85.00
